糟糕有點晚看到這篇（已經寄了QQ）大哈上沒有B12缺乏出現神經症狀的比例只說"Cobalamin deficiency may cause a bilateral peripheral neuropathy" （這邊最好附上網路上找的資料）而在診斷的地方寫說 "The diagnosis of cobalamin or folate deficiency has traditionally depended on the recognition of the relevant abnormalities in the peripheral blood and analysis of the blood levels of the vitamins." 所以應要分析血液方能確診，而非端看有無神經症狀後面folate deficiency治療的地方又說 "Before large doses of folic acid are given, cobalamin deficiency must be excluded and, if present, corrected, otherwise cobalamin neuropathy may develop." 所以沒有確診應不適合單獨給folate 也就是如你所說應該是B+C 建議BC皆可我等下有事沒空去寄，有人還沒寄可以幫忙一下嗎？ ※ 引述《slhoivnee (Mr.青春)》之銘言： : 病患為巨母紅血球性貧血(megaloblastic anemia) 可能情況有缺乏B12與Folate : 雖然題目有說明不具神經性異常~理應為Folate型 : 不過我查這個網站 : http://www.aafp.org/afp/990315ap/1598.html : 在提到Vitamin B12 的第一段倒數第四行那裡~ : Complete or partial resolution of neurologic symptoms occurs in as many as : 80 percent of patients. : 有約80%的人會出現神經症狀 : 換句話說五個人裡面可能有一個人是沒神經症狀症狀型的Vitamin B12缺乏 : 另外在同一個網站提到Folate的最後一段提到 : However, because the use of folic acid supplementation partially corrects the : hematologic abnormalities of vitamin B12 deficiency but not the associated : neurologic deterioration, other experts have recommended that all foods : fortified with folic acid also include vitamin B12 supplementation. : 若只提供葉酸~雖然B12型的症狀會改善~但無法治療神經損傷因此專家們都建議提供 : folate的的同時一併給予B12以防萬一~ : 我質疑的點是~雖然神經症狀是B12型的典型症狀~但是在沒有其它資料的輔助佐證下 : (比方說過去病史，個人史，甚至是性別年齡)似乎難以準確判斷是否真是folate缺乏 : 因此最適當的療法為(B)+(C) : 不過我手邊沒有教科書可以查~感覺這一題有機會... : 可以請板上的大家幫個忙提供一下資料嗎?(或者是幫寄也可以) : 建議是(B) (C)皆給分 : 感謝大家 -- ※ 發信站: 批踢踢實業坊(ptt.cc) ◆ From: 120.126.64.226
