[求譯] 一篇有關於知覺測試的文章
很多字我大概看的懂，但翻譯起來就是不順…
我也不是很確定這篇的主題，大概是在講知覺測試?!
Knowledge of skin segment (dermatome) innvervation by the dorsal roots and
peripheral nerve innervation is required for making sound, accurate
diagnostic and prognostic judgments. they serve as critical references during
testing as well as provide a frame work for documenting results.
試譯: 背部皮膚切割神經(皮節)分佈及周圍神經分布的認識，是需要做合理及
精確診斷的判斷。測試期間它們提供關鍵性的參考也給記錄結果一個架構。
這裡的dorsal roots --->該怎麼翻譯呢?
knowledge 是表示 認識，還是學問? 還是認知?
prognostic 又該怎麼翻譯? 我查出來是寫 預兆後 ?!
下一句的they 指的是什麼? judgments 嗎?
frame work 是什麼意思?
documenting results 又該怎麼翻譯?
Sensory tests are typically performed in a distal to proximal direction.
This progression will conserve time, particularly when dealing with localized
lesions involving a single extremity, where deficits tend to be more severe
distally.
試譯: (第一句看不懂)，此過程節省時間，特別是處理局部的損傷，包括個別的四肢，
(where開始那一句也不懂)
It is generally not necessary to test every segment of each dermatome;
testing general body areas is sufficient. However, once a deficit area is
noted, testing must become more discrete and the exact boundaries of the
impairment should be identified.
試譯: 通常不需測試每一部分的皮節，測試整個身體部位就足夠了。 但若測試到不足
(?) 的部位，測試部位必需分離，能辨識出更精確的損傷範圍。
deficit 要怎麼翻譯?
identified為何要放在最後面? 修飾什麼?
A skin pencil may be useful to mark the boundaries of sensory change
directly. This information should be transferred later to a sensory
examination form, graphically presented on a dermatome chart, and peripheral
nerve involvement identified.
試譯: Skin pencil 可用於直接標示出知覺改變的範圍。 這項資訊之後會做成知覺
檢查表，透過圖表呈現在皮節圖表中，辨識出周圍神經所涉及的範圍。
This form will then be entered in the medical record. Figure 5.11 presents
a sample Sensory Examination Form. Most clinicians find it challenging
to create a single documentation form applicable to the variety of patients
seen in a given practice settings.(with the exception of specialized centers
or clinics.)

However, the following are common elements of sensory examination forms:
1. dermatome chart to graphically display findings. 2. some variation of
a grading scale (e.g., 1-intact; 2-decreased; and so forth) to score patient
perception of individual modalities; and 3.a section for narrative comments.
試譯: 然而，知覺檢查表一般要素為:(1)以圖表呈現發現的皮節圖表 (2)分等級的變化
(例如 1-未受損 2-decreased? 等等)，並為病患個別的物理療法感覺評分
(3) 敘述評論的部分。
