這篇論文是探討病患的 "baseline" SBP,
也就是進行處置之前量到的血壓,
並未評估去拉高或是降低血壓會不會影響結果...
況且 baseline 是怎樣, 這個 case 的命格就是怎樣,
目前還沒有證據支持, 把血壓搞到多少的範圍, 會變比較好命的.
所以那個孫子的觀念是有待商榷的...
但是 normal saline 還是可以給, 基於兩種理由,
第一, 適當的 volume expansion 可能有 hemodilution 的效果,
進而可能增加腦部灌流, 部分研究顯示,
可以降低三到六個月內 DVT, PE, death 之機率.
第二, 點滴掛在架上在視覺上具備 "作秀" 的特質,
符合民眾期待與社會觀感, 可以藉由 placebo effect 增加療效與行醫滿意度,
進而降低進法院之機率.
