Trochlear nerve injury (1093)
CN IV is rarely paralyzed alone
-lesions to nerve or its nucleus = paralysis of superior oblique
and impairment of ability to turn eye inferomedially.
-may be torn in severe head injuries bc of its long intracranial
course
-sign = diplopia (double vision) when looking down. Occurs bc
inferior rectus normally assist the inferior oblique in moving
eye inferiorly, esp when it is in a medial position, thus
direction of gaze is diff for the 2 eyes when attempt to look in
this direction. Person can compensate by inclining the head
anteriorly and to side of normal eye.
