我的申請系統是醫學相關領域網站上有一欄我不大懂意思(每個字都看懂但整體意思不太確定) You must complete the following statement concerning access to student records retained by your university before submitting this form. This request is in compliance with Feferal Law. This selection can't be changed after submission. 1. I waive my right of access to this evaluation. I understand that i will not be permitted to access this evluation. 2.
