[問題] 關於CAH(congnital adrenal hyperplasia)
最近念小兒發現CAH考了不少次(大概一年一次)
CAH常考的有三型
參考steroid製造圖大概可以理解 維基有
http://en.wikipedia.org/wiki/Congenital_adrenal_hyperplasia
但是 11β-Hydroxylase deficiency 中
如果單看圖 aldosterone(minerocorticoid)應該要↑
(因為另外的pathway都不能走才對)
可是在醫院的投影片中放的表格卻說aldosterone會減少(來源不明)
而Nelson的表格提到
Cortisol deficiency (corticosterone is an adequate glucocorticoid)
這也是很奇怪 既然他算是glucocorticoid
那為什麼會cortisol deficiency的症狀呢?
glucocorticoid那條應該就OK啦~
另外 為什麼只會到corticosterone 不會做到aldosterone呢??
而aldosterone的濃度在Nelson中沒提到
參考 Williams的內分泌學
decreased production of cortisol and sex hormones.
low concentrations of plasma adrenal androgens, 17α-hydroxyprogesterone,
aldosterone, and cortisol.
The plasma concentrations of DOC, corticosterone, and 18-OHB are increased
PRA is suppressed.
glucocorticoid replacement normalizes the steroid abnormalities and the
hypertension.

sorry 剛剛才發現我問錯了 @.@
我是想問 17α-Hydroxylase Deficiency
所以這兩個都是 DOC↑ 所以mineralcorticoid activity↑ ==> Hypertension
但aldosterone因為RAA system被抑制 所以都會↓, renin也都是低的囉?
