有個家屬看了報紙後問我說病患能不能接受Left atrial appendage occlusion
該病患為85 y/o man
Afib是最近幾個月新診斷
Paste HX:
HTN with medical control 10+ year
mild to moderate MR
沒有Stroke Hx，不過Brain CT看起來有不少lacunar
mild dementia, BPH, Bil. TKR s/p
病患的ADL來說，每天還能自己去公園走一走
Warfarin是家屬每天按時拿給他吃，沒有non-compliance的問題
經濟上還不錯，自費20萬也能夠負擔
我讀了些Paper
初步的impression是LAAO可以降低Afib病人一半以上的Stroke風險
Procedure的risk也不算太高
但Afib的Stroke風險不完全由Left atrial appendage而來
做完後Warfarin是否還需要續用?
考慮到經濟能力還不錯，自費使用Novel Oral Anticoagulants似乎也可以談
所以選擇大概有:
1. Keep warfarin
2. Shift to NOAC
3. LAAO, then DC warfarin
4. LAAO, then keep warfarin
5. LAAO, then shift to NOAC
etc
Which would be best for the patient?
或者既然要做導管了，考慮ablation?
又，如果我想把病患refer給CV，高雄有哪位醫師有在做LAAO的嗎?或者要讓他上台北?
