[外絮] Reddit討論分析PG傷勢
原文為Reddit上鄉民詢問PG傷勢到底有多嚴重,
然後一位念醫學生跟他其他醫生朋友討論結果。

**I’m not an orthopedist but I am an EM/MD (still in training)
*** Just talked to my Harvard orthopedist friend---she's is quite optimistic,
however, there's just no way to tell based on the grainy video. With that
said, I'll start including bits and pieces of conversations I am having,
although keep in mind it is speculation, and will always be unless you have
the actual patient in front of us. **
This can be classified as a Grade I fracture (since its isolated and clean):
Average time to healing after surgery is 21-28 weeks (5-7 months)
I have a couple of orthofriends looking at the injury: "I'm comparing the two
injuries and PG24's is more distal (further down) and looks frankly like
there was more muscle involvement than Ware's. Ware's was higher up in the
leg, and the bone came out the front of the leg. That makes me think there
was less involvement of the muscle and blood vessels that are crucial for
healing." Thus PGs injury might be worse based on the video because it came
out the back and side of the leg where a lot of crucial blood supply
structures are located.
The issue here is that if the blood supply gets disrupted then the skin
overlaying the bone does too, which stops the bone's ability to heal.
This looks like is an oblique transverse compound (and open) lower limb
fracture.
Since the bone broke the skin, the issue is that he has an increased risk for
infection. This will be the biggest issue for him in the next 24-48 hours.
They will likely take him to the OR and immediately clean out the bone and
remove any muscle that may be torn/damaged.
There is a classification system for this :
http://en.wikipedia.org/wiki/Gustilo_open_fracture_classification
(Skoog: this is a general population study, likely not generalizable to
athletes) Twelve months after the injury, 44 percent had not regained full
function of the injured leg, although all but two of the patients had
returned to preinjury working status.. twelve months after the injury, nearly
half the patients still experienced functional limitations related to the
fracture
Regarding talk of Cameramen, etc, its unlikely that those were the cause. An
unprovoked fracture like this probably had some sort of underlying fracture.
Tibial fractures are far and away the most common type of fracture in
athletes (Shin splits are a form of this). He may have had some sort of small
underlying fracture that gave way when he twisted and landed in an awkward
way.
Yes, Kevin Ware had the same injury, but so did Joe Namath and this injury
ended his career. And I'd argue that Kevin Ware was never the same again, and
also was not the elite level athlete that PG was (360 windmill dunks are not
something that Kevin Ware could do before the surgery). We may never see the
same athleticism out of PG again
I, like everyone else here, hurled a little bit when I saw that play. I'm
researching articles to see if I can find anything that would give a clear
idea of his recovery. However, let's be clear here: the number of elite
athletes with complete transverse open tibial fractures is like...n=5.
There's not a lot of data for how these patients do.
I'm looking specifically for anything specifically for athletes, and
excluding trauma for these sorts of injuries (car accidents). What's
difficult is that this injury basically depends on 3 things: 1) blood supply
2) resource/rehab of the patient 3) if any other muscles were torn (looks
possible in that video.)
At this point, its unlikely that anything more can be added to the
conversation unless more information comes out. Its just speculation and
frankly, MD's can't really say much about this unless we have the Xray and
open him up ourselves, because these injuries in >99th percentile athletes
just don't happen very often.
** If any orthopods read this, please PM me **
簡易版:
TL;DR:
-Likely 5-7 months
-We're talking a range of return next season to career ending, depending on
the associated damage
-Most likely possibility: He gets surgery, avoids infection, and starts rehab
in 2014-2015. He returns to form somewhere between 2015-2016. Paul George
Never gets his athleticism, but is an effective player: 2016-2017.
As a Bulls fan, this makes me sick
