  Log:
  Split inline programlisting elements out of paragraphs.  Correct content
  for many of those elements which had spaces instead of tabs and extra
  line breaks.  Also add a couple of missing </entry> tags and switch the
  service(8) example in the pre-commit checklist to use <screen>.
