請問這題為什麼不能用if

我看了網路上的例句， Unless 跟 if 意思相反，跟if not 意思相同
Come tomorrow unless I call. = Come tomorrow if I don't call.
如果將題目的正確答案unless 換成 if not會變成
There was little hope, he said, if proper medicine couldn't be found
如果沒有找到適合的藥，就有一點希望
這樣不就跟原題目的"除非找到適合的藥，就有一點希望"相反了嗎
請問這題答案是不是有問題
