kernel - VM PAGER part 1/2 - Remove vm_page_alloc()
* Break out the individual VM object allocation functions and remove
the vm_pager_alloc() ops vector.
* This is in preparation for giving the vnode_pager_alloc() some
additional arguments to match recent buffer cache work.
