HAMMER - Stabilize and refactor volume removal
* Correctly handle already freed layer1 entries.
* Correclty avoid reallocation on the volume to be removed
during reblocking.
* Erase volume header after removal so that trying to mount it
afterwards will fail (otherwise mount will silently use it).
