Brief History
The patient had a right hemorrhage stroke of the brain when he was travelling
in Japan. When the stroke struck, he heard a "slap sound" in his head, and
then he found himself incapable of moving his left side. He was then sent to
a Japanese hospital, with consciousness intact. He came back to Taiwan on
Jul 9 2009 for further treatment.
He had some MRI examinations in Japan and Taiwan as well, of which a hemorrhage
stroke with blood clot left on the thalamus was confirmed. He's now taking
medicines to control blood pressure, blood sugar and blood fat.
OBJECTIVE FINDINGS
No significant findings
MOTOR FUNCTIONS
A. Muscle tone: Flexor and extensor tone of biceps: 2
Dorsiflexor and plantar flexor: 2
B. Muscle strength: U/E general of 5 with wrist extensor 3 and flexor 2
L/E general of 4 with hip flexor 4 extensor 3
C. DTR: biceps +++/knee jerks +++
D. Brunnstrom stage
(1)U/E: 5 - pronation and supination 0
(2)L/E: 5 - dorsi/plantar flexion 0
(3)hand: 3 - 0
